Isaiah Pulu, a 25-year-old driver for a Utah transportation service aiding disabled adults, faces three counts of manslaughter and three counts of aggravated abuse of a vulnerable adult after three men died from carbon monoxide poisoning on February 6, 2026, in West Valley City.
Incident Details
Pulu reportedly stopped at his apartment around 10 a.m., leaving Colton Moser (25), Mosa’ati Moa (22), and Tim Jones (39)—residents of assisted living facilities—in a running van with windows down and garage door open while he ate lunch and watched TV for over three hours. He returned around 1:30-2 p.m. to find the garage door closed, van off, and all three men unresponsive and deceased; two were non-verbal, and one had been disruptive.
Charges and Arrest
Booked into Salt Lake County Jail without bail, Pulu admitted to police that one passenger was combative, leading him to leave them; first responders pronounced the men dead at the scene near 2700 S High Park Way.
Agency Response
The Utah Department of Health and Human Services issued a 30-day emergency suspension to Safe and Sound Services, LLC’s day treatment license pending investigation, emphasizing commitment to client safety.
